AGREEMENT
It is agreed and understood that:
It is the responsibility of the renter of the Center to leave the building in as neat and clean condition as it was in the beginning of the term of use. Cleaning supplies will be made available for the renter to maintain the cleanliness of the Civic Center. It is the renter’s responsibility to ensure that all cleaning supplies remain in the Center. Any conditions to the Center that are found by the renter at the beginning of the term shall be immediately reported to the City Dispatcher within one (1) hour or said conditions will be the responsibility of the renter.
Renter also assumes full responsibility for the care of the building, furnishings, plumbing fixtures, and surrounding premises against all damage and breakage, and will not deface the walls or any portion of the building using nails, tacks, etc. In addition, for verification purposes, furnishings (tables and chairs) do not need to be returned to the storage closet. It is further expressly acknowledged that the renter is responsible and liable for all damages caused to the Center during the term of usage, whether such damage occurs by the negligent acts of the renter’s guest, or whether such damages occur by any act other than the “ACT OF GOD”.
Alcoholic beverage use can occur inside the Center only if the renter contracts with an off-duty Lacy Lakeview Police Officer to serve as security for such a function. The renter will contact the Lacy Lakeview Police Department to make such arrangements. The off-duty officer will be paid hourly by the renter. The Lacy Lakeview Officer will be paid an hourly rate of fifty (50) dollars per hour. The officer will notify City Hall as soon as practicable to confirm he/she will be providing security. At no time will drinking alcoholic beverages be allowed outside the Civic Center on City property.
This agreement and City Ordinance prohibit smoking inside the center. It is the responsibility of the renter to ensure that no smoking is allowed inside the Center during the term of use agreed upon herein with the City.
§ 99.41 – Smoking prohibited areas.
A. Smoking is hereby prohibited in the following non-enclosed public places: parks and playgrounds as defined in this subchapter, and at public events held on property owned or occupied by the city and used for city purposes.
B. Smoking is hereby prohibited in all enclosed areas that are owned or occupied by the city and used for city purposes, including, but not limited to, buildings and vehicles. In addition, the City Manager may designate any non-enclosed areas owned or occupied by the city and used for city purposes, as nonsmoking. Smoking is prohibited in those designated areas.
C. Smoking is hereby prohibited in any enclosed public place, establishment, facility, place of employment, or business that the owner or operator has declared a portion of, or that entire establishment, facility, place of employment or business as nonsmoking and has posted the signage required in § 99.43 below.
D. Smoking is hereby prohibited in any non-enclosed public place, establishment, facility, or business that the owner or operator has declared a portion of, or that entire establishment, facility or business as nonsmoking and has posted the signage required in § 99.43 below.
E. Smoking is hereby prohibited at any place within 15 feet of any entrances, windows, ventilation systems or any other openings of an enclosed area where smoking is prohibited.

Periodically, officers from the Lacy Lakeview Police Department will ensure that all Rules are being followed. If an officer discovers the renter to be in violation of items #3 or #4 above, a citation may be issued, and termination of the event will occur.
Main parking for the Center is on Crescent Street side. Parking is available for a limited number of vehicles on the west side of the center with the agreement that vehicles do not block the Fire Department doors. Vehicles blocking the Fire Department doors will be towed away at owners’ expense.
Renter agrees to ensure that all activities with the Center during the rental period will always be supervised by responsible adults and they are always present.
Commercial sales and/or solicitations will be strictly prohibited. This will include, but not limited to garage sales, whether for profit or non-profit.
At the end of the agreed terms of use, the renter will securely lock all doors; turn off the water, and all lights unless otherwise instructed by the City.
The key to the Center will be returned to Dispatch promptly after the terms of the rental agreement are met.
I, the undersigned have read, understand, and will abide by the rules set forth by the City of Lacy Lakeview and the Civic Center and take full responsibility for any damages, injuries and/or death that occur during my event.
