REQUEST FOR DEFERRED DISPOSITION FOR A VIOLATION DISMISSAL
            (Failure to return this form on or before your appearance date will result in ineligibility)
            Deferred Disposition is a 90 or 180 day probationary period giving you the opportunity to keep the violation off your driving record. On your plea of guilty or no contest, the Judge will defer the finding of guilt until a later date, assess the applicable court cost and fees, and order you to comply with certain conditions. If you successfully comply with the terms, your case will be dismissed.
            If you fail to comply with the conditions, a final judgment will be imposed and a conviction will be reported to DPS.
            I understand I am not eligible for deferred disposition if I
            
              - Am currently on Deferred Disposition for another violation of the same nature in this court;
- Hold a Commercial Driver License;
- The citation resulted from a traffic accident;
- A second or subsequent offense of Failure to Maintain Financial Responsibility (No Liability Insurance);
- Have committed one of the following offenses;(A) Failure to Give Information at Accident Scene; 
 (B) Leaving scene of accident;
 (C) Fleeing or attempting to elude police officer;
 (D) Reckless driving;
 (E) Passing a school bus;
 (F) A serious traffic violation;
 (G) An offense in a construction maintenance zone when workers are present;
 (H) Speeding 25 mph or more over limit;
 (I) Offense involving Alcohol or Tobacco;
 (J) Offense involving Assault, Disorderly Conduct, or Theft.
